Integrated Community Care Provider (ICCP)

As Singapore approaches super-aged status by 2026, NTUC Health and our partners are working with the Ministry of Health (MOH) and the Agency for Integrated Care (AIC) to progressively implement a place-based approach to support seniors in the community.

What is ICCP?

To coordinate different community care services more effectively, the sector has been re-organised to support seniors in smaller sub-regions. The providers will form a network to coordinate their services and support to the seniors working with health and social care providers within the sub-region to form an integrated support network for seniors. This network of providers is known as the Integrated Community Care Provider (ICCP).

How ICCP Helps Seniors

The ICCP model ensures seniors receive timely, appropriate, and well-coordinated care. 

Through this approach, we aim to: 

  1. Keep seniors socially connected and active through Active Ageing Centres
  2. Identify and support seniors-at-risk earlier through preventive health and social care interventions
  3. Deliver timely and coordinated care as seniors’ health and social needs change
