The Early Recovery After Surgery (ERAS) programme was was pioneered by Singapore General Hospital (SGH), in collaboration with Tan Tock Seng Hospital (TTSH), to facilitate early discharge and home-based recovery for patients after a total knee replacement (TKR) or total hip replacement (THR) surgery.
This programme has significantly reduced a patient’s length of stay from the usual 4-5 days to 1-2 days, freeing hospital beds for more critical cases.
NTUC Health has been part of ERAS since 2022, supporting over 2,600 clients from both SGH and TTSH across Singapore to recover in the comfort of their homes. Over 92% of all our clients are able to ambulate independently with or without walking aids in their first home therapy visit.
The Stroke Rehab Ecosystem is a Lien Foundation-funded initiative led by the Rehabilitation Medicine Departments at Singapore General Hospital and Tan Tock Seng Hospital, in collaboration with community partners including NTUC Health.
Launched in 2025, the initiative seeks to enhance post-stroke community care by improving timely access to community rehabilitation, streamlining hospital-to-community referral pathways, providing home therapy bridging support, and establishing escalation protocols for post-stroke complications so that patients can receive appropriate care earlier.
The initiative also aims to strengthen the capabilities of community therapists through training and upskilling in areas such as mental health and wellness, early detection, aphasia communication strategies, and targeted upper limb rehabilitation interventions.


CONNACT Back is a collaboration between NTUC Health and Woodlands Hospital that started in 2026. The programme is designed to transform the care journey for people living with lower back pain by building confidence and resilience through structured, coaching-based support focused on improving your pain and physical function.
Our collaborative care model creates a seamless referral pathway. Patients from Woodlands Hospital and Polyclinic are transitioned to NTUC Health's Day Rehabilitation Centre (DRC) and Senior Gym. The journey begins with a comprehensive assessment to tailor the support plan to each individual's needs.
Suitable clients join the CONNACT Back in-person programme, where trainers guide them to improve strength, functional capacity and confidence. Beyond structured exercise, these sessions reinforce key principles of self-management including goal-setting, pacing and long-term condition management. Trainers are equipped with resources to support the self-management journey.
Together, this integrated approach enhances continuity of care, supports self-efficacy, and enables sustained engagement in recovery beyond the hospital setting.
