Those who meet the criteria will receive an SMS from MOH inviting them to enrol with a Healthier SG clinic via the HealthHub app.
If you haven’t received your SMS yet, you may request for the SMS invitation to be sent by submitting your details here.

Follow these steps to enrol in Healthier SG with NTUC Health Family Medicine Clinic.
Check your SMS invite from MOH. Click on the link to download and launch the HealthHub app.
You may also enrol via the HealthHub web portal or download the HealthHub app.
Login with your Singpass.
Tap on the Healthier SG banner to begin your enrolment.
Tap on your name to enrol.
Continue with the enrolment process by following the next-steps.
Tap on "Select preferred clinic" when you see the page on Step 5 in the image below.
Choose NTUC Health Family Medicine Clinic as your preferred Healthier SG clinic.
You can then search for us by tapping on the Search/Filters button as seen in Step 6.
You may also approach our clinic for assistance with on-site enrollment.
Tap on "Confirm enrolment" to complete your enrolment.
You can then schedule your first Healthier SG Health Plan consultation with us by either booking an appointment via the HealthHub app or calling our clinic at 6281 2638.
By enrolling in Healthier SG, you can follow through on your family doctor’s advice and Health Plan by completing the recommended health screenings, immunisations, and lifestyle adjustments, as well as track your progress on HealthHub.
Enrolment is not compulsory, but we strongly encourage it.

Here are three easy steps to book your first Health Plan consultation with us:
Step 1:
Log in to the HealthHub app using your Singpass.
Tap the green 'Healthier SG' banner on the homepage or find 'Healthier SG' under the Service Categories page.
Step 2:
Follow the app’s instructions to complete your Healthier SG enrolment.
Step 3:
Once enrolled, you’ll be directed to the 'Book appointment' screen. You can also go to the Health Recommendations page and select 'Manage appointment' to schedule your consultation.
From there, you can book your appointment through:
Absolutely! With their consent, you can help your family members enrol in Healthier SG and even access their health plans through the HealthHub app. You’ll also be able to view their medical reports, and make appointments for them via the same app.
To enrol them, you will need their Singpass account for authorisation via HealthHub. For detailed guidance, please refer to this step-by-step enrollment guide for caregivers.

If you're enrolled in Healthier SG, and a Singapore Citizen or Permanent Resident, you're eligible to receive full subsidies for screenings and vaccinations recommended under two key health programmes: the Screen-for-Life (SFL) and the National Adult Immunisation Schedule (NAIS).
Screen-for-Life (SFL)
The SFL programme covers nationally recommended screening tests for cardiovascular disease, diabetes, high blood pressure, high cholesterol, as well as cervical, colorectal, and breast cancers.
National Adult Immunisation Schedule (NAIS)
The NAIS covers nationally recommended vaccinations, such as pneumococcal and annual flu vaccines, for seniors aged 65 and above.
Healthier SG enrollees can check for their SFL eligibility and learn more about NAIS on HealthHub.
Upon enrolment in Healthier SG, you gain access to commonly used medications for managing chronic conditions like diabetes, high blood pressure, and high cholesterol at significantly reduced prices.
These medications will be eligible for percentage subsidies up to 87.5%, making them more affordable at your enrolled Healthier SG clinic, with prices similar to those at polyclinics. You can find the complete list of eligible chronic medications here.
Even if your medication isn't listed, you can still receive dollar-based subsidies ranging from $28 to $135 per visit if you have a CHAS, Merdeka Generation (MG), or Pioneer Generation (PG) card. The exact amount will depend on your eligible annual cap based on your card status.
Upon successfully completing your first Health Plan consultation, you’ll receive 3,000 Healthpoints (valued at $20) credited to your Healthy 365 app account.
If you don’t have a Healthy 365 account yet, make sure to create one within 12 months of your consultation, otherwise the Healthpoints will expire. Here’s a video guide on how to sign up for your Healthy 365 account.
You can only access Healthier SG benefits and subsidies at your enrolled Healthier SG clinic.
