The CREST team focuses on raising public awareness of mental health conditions including dementia, promoting early recognition of at-risk individuals, providing emotional support to individuals and their caregivers, and connecting individuals to relevant health and social care services.
Programme & Objectives
CREST works closely with Community Partners and Primary Care Providers to coordinate services and ensure continuity of care for clients with depression or dementia.
- Increase awareness of mental health and dementia by organising outreach events to provide mental health information and education to residents and caregivers in the region.
- Promote early identification of signs and symptoms of mental health conditions and dementia.
- Provide holistic and integrated care for clients and caregivers through basic emotional support, monitoring, service linkage and meaningful activities.
- Network and collaborate with health and community care partners and key stakeholders to enhance support for those with mental health conditions and their caregivers.
Who We Support
Our services are available to individuals aged 40 and above who may require mental health or dementia-related care and are residing in the following areas:
- Bukit Merah
- Downtown
- Jurong West
- Pioneer
- Tanglin
Individuals must meet at least one of the following conditions:
- Singapore Citizens or Permanent Residents seeking professional mental health support.
- Individuals experiencing mental health challenges, including depression, anxiety, bipolar disorder, obsessive-compulsive disorder (OCD), psychosis, or other conditions.
- Individuals at risk or showing early signs of dementia who require assessment and support.
- Caregivers who need guidance and assistance in managing the care of loved ones with dementia or mental health conditions.
