The CREST team focuses on raising public awareness of mental health conditions and dementia, identifying at-risk individuals early, providing emotional support to clients and caregivers, and connecting families to appropriate healthcare and social services across Singapore.
Programme & Key Objectives of CREST
CREST works closely with Community Partners and Primary Care Providers, ensuring seamless, continuous care for individuals facing mental health challenges or memory loss.
- Community Outreach & Education: Organising outreach events and workshops to provide mental health information and education to residents and caregivers in the region, increasing awareness of mental health and dementia.
- Early Identification: Screening residents early for signs of depression, anxiety, or cognitive decline.
- Holistic Client & Caregiver Care: Offering basic emotional support, active monitoring, and engagement through meaningful community activities.
- Integrated Care: Navigating Singapore's health and social care systems to connect clients with the right care providers, to enhance support for those with mental health conditions and their caregivers
Who We Support (Eligibility)
Our services are available to individuals aged 40 and above who may require mental health or dementia-related care and are residing in the following areas:
- Bukit Merah
- Downtown
- Jurong West
- Pioneer
- Tanglin
Individuals must meet at least one of the following conditions:
- Singapore Citizens or Permanent Residents seeking professional mental health support.
- Individuals experiencing mental health challenges, including depression, anxiety, bipolar disorder, obsessive-compulsive disorder (OCD), psychosis, or other conditions.
- Individuals at risk or showing early signs of dementia who require assessment and support.
- Caregivers who need guidance and assistance in managing the care of loved ones with dementia or mental health conditions.
