Community Case Management Services

Our social workers help vulnerable seniors improve their quality of life by supporting their physical, social and mental care needs. We listen to their concerns, help them overcome daily challenges and connect them to community resources so they can live at home for as long as possible.  

Programme & Objectives
Community Case Management Services supports vulnerable seniors and their caregivers who have challenges managing complex health and social care needs, enabling seniors to age in place with dignity.

The programme aims to prevent or delay seniors from moving into institutionalised care homes by:

  • Addressing clients’ holistic care needs, including medical, functional, behavioural, and psychosocial aspects
  • Applying evidence-based guidelines in practice to improve clinical outcomes
  • Coordinating services and support that aligns with clients’ goals
  • Assisting clients and caregivers to navigate care options and support resources
  • Promoting appropriate use of community resources
  • Advocating for clients’ best interests
  • Improving the client’s quality of life and caregiver’s satisfaction


Who We Support
Our services are available to seniors aged 60 and above, who reside in the following areas:

  • Boon Lay
  • Bukit Merah
  • Downtown Core
  • Jurong East
  • Jurong West
  • Newton
  • Orchard
  • Outram
  • Pioneer
  • River Valley
  • Singapore River
  • Tanglin
  • Tengah

Seniors must meet at least one of the following conditions:

  1. Require assistance with daily living activities such as mobility, feeding, transferring, dressing, bathing, or toileting
  2. Unable to perform and require help with household tasks like light housekeeping, meal preparation, medication management, grocery shopping, phone usage, or financial management
  3. Have little to no family or caregiver support
  4. Experience self-neglect, abuse, mistreatment, social isolation, low mood, or suicidal thoughts
  5. Show signs of or have been diagnosed with a mental health condition
  6. Require assistance with hoarding, decluttering, or home improvements to ensure a safe living environment

Our Services

  • Client Engagement
    • Work closely with clients to understand their concerns and goals, and provide the necessary support.
  • Caregiver Education, Support, and Counselling
    • Share information with caregivers on community services, resources, subsidy schemes, government policies, etc.
    • Provide emotional support to caregivers, including para-counselling and support group activities.
  • Care Coordination
    • Assess clients' care needs and connect them to relevant community resources such as meal delivery, assistive devices, home therapy services, home personal care services, monitoring medical compliance, arranging of day care or institutional care, befriending, financial assessment, medical escort transport, etc.
  • Home Improvement and Environmental Safety
    • Assess and identify obstacles that increase the risk of falls at home
    • Arrange for home modifications to make the living environment safer and to aid rehabilitation. This includes decluttering, cleaning, fumigation, HDB grab bars installation, and anti-slip floor protection, etc.
