DISABILITY ASSESSMENT

Receive a disability assessment at our Family Medicine Clinic to help with applications for financial aid

Did you know that you may be eligible for financial assistance schemes like Eldershield and Careshield Life if you are caring for someone who needs help with activities of daily living such as bathing, toileting or meals? To apply for financial assistance, just call us for a medical evaluation that is required to support the application.

NTUC Health is an MOH-accredited assessor for disability assessments, and we will not only help determine your eligibility for financial support, but also help you to better understand and plan for your long-term care needs.

WHAT TO EXPECT

Receive a professional evaluation at NTUC Health Family Medicine Clinic, or opt for a house call to undergo an assessment. A disability assessment evaluates your ability to perform the following 6 activities of daily living without help:

  • Eating
  • Bathing
  • Getting dressed
  • Using the toilet
  • Walking or moving around
  • Transferring out of bed or a chair

With a disability assessment, you will receive the completed assessment report and be able to apply for the financial support that you are eligible for. Speak to our doctors or clinic staff to find out more about disability assessments, or get in touch with us at 6281 2638 for more details.
