SENIOR CARE CENTRES

Silver Circle currently operates 14 Silver Circle senior care centres. At the centres, the elderly engage in interactive activities, such as regular exercises, interesting games and festive celebrations, together with their peers while their family members are at work in the day.

In addition to social day care services, Silver Circle Senior Care Centres are integrated day care facilities that also offer enhanced care services such as community nursing, active rehabilitation and community dementia care services in one convenient location. To support caregivers who need to take some time off on weekdays or weekends for urgent work commitments, or to run errands, respite care service is also available at selected centres.

Transportation can also be arranged at a nominal cost. Government subsidies and financial assistance are available for eligible clients.

MAINTENANCE DAY CARE

  • Provides day care for seniors while caregivers are at work
  • Social activities

DEMENTIA CARE

  • Structured dementia programme
  • Group and individual activities
  • Support for caregivers of elderly with dementia

COMMUNITY NURSING

  • Continence management
  • Medication administration
  • Nutritional management
  • Support with activities of daily living
  • Wound management

ACTIVE REHABILITATION

For elderly who may have had the following:

  • Strokes and fractures
  • Spinal cord injuries
  • Parkinson’s disease
  • Recent operations requiring follow-up rehabilitation such as total knee replacement

WEEKEND RESPITE CARE

  • Only at Serangoon Central Senior Care Centre
  • Saturday
    9.00am – 5.00pm (Only for Serangoon Central Senior Care Centre weekend respite care)

TRANSPORT

  • Available based on locations (within 4km from Day/ Senior Care Centre)

Government subsidies and financial assistance are available for eligible clients.

7:30am – 6:00pm
Saturday
9:00am – 5:00pm

ADMISSION CRITERIA

Enrollment is open to elderly who are:

  • Ambulant or semi-ambulant
  • Able to conduct their activities independently
  • Not suffering from severe mental illness, behavioural disorders or contagious diseases

Applications who fall outside the above criteria (e.g. wheelchair-bound) will be considered on a case-by-case basis.

DOCUMENTS REQUIRED

  • Recent medical report (e.g. discharge summary/ in-patient summary from hospital, or memo from General Practitioner)
  • Chest X-Ray Report (within 6 months from the report)
  • Referral Document from Agency for Integrated Care
